Providing a cure for people with hepatitis C
In the last 2 years we have seen a huge revolution in providing treatments for patients with chronic hepatitis C. Historically treatment was curative in fewer than half of patients and required up to 1 year treatment with interferon injections which are poorly tolerated and have high rates of physical and mental health side effects.
The new treatments consist of well tolerated tablets for 8-12 weeks with a cure rate of over 95%. However, deciding treatment is complex- based on type of hepatic C, previous treatment and severity of liver disease. As expected due to the complexity and the expense of the treatment (approximately £30k) the treatment is tightly (and rightly) regulated by NHS England.
An enormous amount of admin time has been involved in setting up pathways with partner trusts to ensure equity of access, standardised treatment and monitoring, and careful evaluation of outcomes. The complexity has been enhanced by continual changes in medication as new drugs enter the market and prices change. At the moment the treatment regimens are updated every three months,
The model of care for patients with hepatitis C has been revolutionised from one which is based on monitoring for complications of chronic hepatitis (screening for liver cancer and cirrhosis) to one geared up to expanding testing and delivering treatment to maximise cure. This reduces risk of complications and – most importantly – the onward spread of infection
Before the new treatment, we treated 30 patients a year. Last year 143 patients have been treated with over 96% cured which is a huge success!
However, we know that having hepatitis C does not provide immunity for life and patients can get re-infected especially if the risks for acquisition in the first place are not addressed.
With this in mind, we have implemented a peer run workshop to help you stay hepatitis C free. Up to 25% of patients may get re-infected within 2 years. So far, we have only one patient (0.5%) with a confirmed re-infection!
We are taking diagnosis and treatment into the community with outreach clinics and cooperation of drug/alcohol clinics, hostels and prisons We have established several pathways for patients who find it otherwise difficult to access services getting assessed and treated..
